Декомпенсированный криптогенный цирроз печени: клинический случай

Цирроз печени – актуальная проблема клинической гастроэнтерологии. Он отличается длительным малосимптомным течением, нередко внезапной декомпенсацией и высокой смертностью. Цель: представить клинический случай декомпенсированного криптогенного цирроза печени и проанализировать современные данные о его возможных причинах, естественном течении, диагностике, лечении и исходах. Материалы и методы. Проведено клинико-лабораторное и инструментальное обследование пациентки 42 лет, включая ультразвуковое исследование органов брюшной полости, компьютерную томографию, эзофагогастродуоденоскопию, а также анализ современной отечественной и зарубежной литературы. Результаты. У пациентки выявлены асцит, портальная гипертензия, спленомегалия, гиперспленизм, варикозное расширение вен пищевода и печёночная энцефалопатия II степени. Вирусные гепатиты, злоупотребление алкоголем, аутоиммунные и наследственные заболевания печени были исключены, что позволило установить диагноз криптогенного цирроза печени. Заключение. Криптогенный цирроз печени является диагнозом исключения и нередко представляет собой конечную стадию ранее нераспознанной метаболически-ассоциированной жировой болезни печени. Ранняя комплексная диагностика и своевременная коррекция осложнений имеют решающее значение для прогноза.
